INSURANCE & REIMBURSEMENT

Out-of-network
care.

This is a private-pay, out-of-network practice. You are responsible for the practice’s fees. Any private-insurance reimbursement depends on your plan’s benefits and claim requirements, and is not guaranteed.

Before scheduling, ask your insurer about out-of-network mental health benefits, the deductible, coinsurance, and documentation requirements.

  • Ask whether your plan includes out-of-network outpatient mental health benefits, what deductible and coinsurance apply, how the allowed amount is calculated, and whether authorization or referrals are required. Ask which documents are needed, how to submit a claim, and what deadlines or limits apply. Benefits and reimbursement are determined by your plan; payment by an insurer is not guaranteed.

POLICIES TO CONFIRM

Ask before
you schedule.

Please confirm these arrangements with Dr. Debiec before scheduling paid care. The appointment rates alone do not establish these policies.

Payment & cancellation

Confirm when payment is due, accepted payment methods, and cancellation or missed-appointment terms before scheduling.

Superbills & claim documentation

Confirm whether a superbill can be provided and which documents your insurer needs. Documentation does not guarantee reimbursement.

Medicare & Medicaid

Confirm participation, reimbursement eligibility, and any implications for prescription coverage directly with the practice and your plan before scheduling.

Services beyond the listed visits

Ask whether letters, forms, school meetings, or other services involve additional fees before arranging them.

UNINSURED OR SELF-PAY PATIENTS

Your right to a
Good Faith Estimate.

If you are uninsured or plan to pay for care without using insurance, you can request a written Good Faith Estimate of expected charges before scheduling care. An estimate is generally required for services scheduled at least three business days in advance.

If a provider’s bill is $400 or more above that provider’s estimate, you may qualify for a federal dispute process. Keep a copy of the estimate.

BEFORE YOU DECIDE

Questions before
getting started?

The free introduction is a chance to discuss fit and ask initial questions before deciding whether to proceed with paid care.

Free 15-minute introduction by phone or video. Not an evaluation or treatment appointment. A request is not a confirmed appointment.

An inquiry does not establish a treatment relationship. Do not send urgent concerns or detailed clinical records through the public consultation form.
